    Congo Ebola outbreak grows faster than past epidemics

    DEMOCRATIC REPUBLIC OF THE CONGO / RankWire.AI / – Congo’s Ebola outbreak had reached 3,874 confirmed cases and 1,751 deaths by August 3, making it the country’s largest recorded epidemic. It ranks second worldwide only to the 2014 to 2016 West Africa outbreak. Congo crossed 1,000 confirmed cases within 40 days after activating its response. Its 2018 outbreak took about 235 days to pass the same mark. The rapid rise reflects delayed detection, weak surveillance, conflict, mobility, and the absence of approved strain-specific medical tools.

    Congo’s Ministry of Public Health declared the outbreak on May 15 after testing identified Bundibugyo virus in Ituri province. WHO first received an alert on May 5, following reports of a deadly unexplained illness around Mongbwalu. Later investigations found that the virus had circulated for months before officials recognized the outbreak. Initial tests in Bunia did not identify Bundibugyo, while early symptoms resembled malaria and other common febrile illnesses. That delay allowed infected people and contacts to move through communities before isolation and tracing expanded.

    The virus species also changed the available response. Licensed Ebola vaccines and proven antibody treatments target Zaire ebolavirus, which caused Congo’s 2018 to 2020 epidemic. No approved vaccine or specific treatment exists for Bundibugyo virus disease. Patients therefore depend on early diagnosis, isolation, supportive care, infection control, contact tracing, and safe burials. The World Health Organization has added a Bundibugyo diagnostic test to its emergency list and started treatment studies, but those steps came after transmission had spread widely.

    Delayed detection overwhelmed contact tracing

    The outbreak has expanded from Mongbwalu to 49 health zones across Ituri, North Kivu, South Kivu, Haut-Uele, and Tshopo. Ituri accounts for most infections and deaths, with Bunia, Rwampara, and Mongbwalu among the hardest-hit health zones. WHO tracked 17,863 contacts by July 30, but only about three quarters received active follow-up in several affected provinces. Health officials also report that most new infections fall outside known contact chains. Surveillance teams are finding many patients only after further exposure has occurred.

    Conflict and displacement make that surveillance harder. Armed attacks have restricted access, interrupted response work, and forced some health teams to suspend operations. Mining routes, trade corridors, crowded displacement sites, and cross-border travel keep large numbers of people moving through affected areas. Health facilities also face shortages of protective equipment, laboratory access, transport, and trained staff. By July 30, Congo had recorded 151 infections and 44 deaths among health workers. Front-line staff have also stopped work in some locations over delayed or inadequate pay.

    Conflict and treatment gaps strain containment

    Ebola spreads through direct contact with the blood or body fluids of a sick or deceased person. It does not spread like influenza through casual proximity. Transmission rises in clinics without strong infection controls and during burials involving contact with infected bodies. More than 60% of recent deaths occurred outside treatment centers, complicating safe burial and contact investigations. Congo’s health authorities, WHO, and Africa CDC have expanded laboratories, treatment centers, community outreach, and border surveillance. The response, however, still trails the scale and speed of new infections.

    Uganda ended its linked outbreak on July 28 after 42 days without a new locally transmitted case. No secondary transmission followed the single case treated in France, and the patient recovered. Congo remains the center of sustained transmission, with a confirmed case fatality rate of about 45% in early August. The outbreak is spreading faster because detection began late, contact tracing misses many chains, and insecurity limits access. Bundibugyo’s lack of approved vaccines and treatments removes tools that helped contain earlier Zaire Ebola epidemics. Together, these factors explain the unusually rapid case growth.
